Advocate Medical Group Amundsen School Health Center Amundsen School Health Center is a partnership between Advocate Illinois Masonic Medical Center, Advocate Medical Group, and Amundsen High School. The goal of the Health Center is to improve the physical and emotional health of Amundsen students and teach them life-long positive health behaviors. Students must have a signed parental/guardian consent form on file before they can receive services at the Health Center. During the 2008-2009 school year, Amundsen Health Center provided nearly 2300 individual office visits and had over 5000 student contacts through group visits. More than 90% of students who attended Amundsen HS had a signed parental consent form on file. Health Center staff are employees of Advocate Medical Group. Providers include Family Physicians, Nurse Practitioners, Clinical Psychologists, Health Educators, and a Medical Assistant. The Health Center is open for students Monday through Friday during school hours. The staff of the Health Center considers parental involvement important. Every student is encouraged to involve their parent(s)/guardian(s) in health care decisions. Download the Frequently Asked Questions (FAQ) for more information.
